As per the terms of your appointment letter, you had to appear for a medical examination and provide your medical records within 15 days of your joining. However, it has been brought to our attention that you have not visited the Company’s doctor on the date and time scheduled for you. This is against the terms of the contract.
We have arranged an alternate appointment for you. Your absence from the same will lead to the termination of your employment contract with ABCL ltd and will lead to a legal action against you. The details of your appointment are as follows:

I was treated at your hospital, from 4th February to 16th February under the treatment of Dr. ABC. I request you to mail a copy of all my medical records at my mailing address.
I do not belong to your town where I was staying only for my treatment. I was diagnosed of (mention the illness) and was recommended to your hospital by the doctors of my town (mention name of town). Though most part of my treatment was done at your hospital between the dates I mentioned above, yet I was advised to seek medication and examination from time to time from a doctor of my town.
While coming back to my hometown, I seem to have misplaced my medical report file. I tried searching for the file wherever I possibly could but to no result. For further consultation and treatment, I urgently require a copy of all my medical records as generated by your hospital.
I understand that you shall be charging me for an additional copy of the reports but I believe charges shall not be applicable for the time required in generating the reports. I am facing certain problems without my medical records as the doctors here are not ready to prescribe further medication without the test reports and the records of treatment.
I also understand that postal charges shall be applicable for preceding my request. I shall be grateful if you could send me the medical records via speed post. Kindly let me know the total charges required for the entire process. I shall immediately send the amount and would appreciate if the same could be done from your side regarding my reports. My mailing address and contact number is mentioned below.
