This is in regards to my health insurance Policy number 8638474948. I Smita Julka have registered for your policy “Pure Health” on 17th July, 2011, which I have been renewing every year without failure. Nine months back I met with an accident wherein all my treatment was covered under the registered policy. As per the procedure, I have taken all the necessary steps in depositing the required documents in order to claim for amount incurred. From the diagnosis to the medical billing, all of them were deposited with utmost priority.
In return for the claim submitted, I was delivered the form for explanation of benefits wherein all the expenses incurred and the reimbursement were being clearly mentioned. Few of the claims were denied, which I agreed upon but majority of them were being accepted and approved by the authority to get it processes further. It was last week that I got the cheque for amount claimed, which was to my surprise not matching the amount that was agreed upon. I have even tried connecting one of your customer care person who can help me rectify the error made by your organization. After making all possible attempts, I went to one of your offices in town. It was very disheartening to see the behavior of the agents who actually reach on time to collect your premiums without fault. Finally, one of your agents helped and spared his time listening to me. As suggested, I have again submitted all my documents.
It will be highly appreciated if you can rectify the errors made in the cheque issued, as it is almost a year now that I am struggling to get my claim.

I am writing to inform you that the cheque I have received against my car insurance claim has a significant error. The amount mentioned is not correct since it fails to cover the repair expenses.
I had filed a claim on the car insurance I have with you since I needed it to cover the costs for the repair of my Toyota Corolla which was damaged during the floods in Mumbai. I had attached the repair estimates made by four different car companies. All details were included.
However the cheque received is not sufficient to go for repairs.
Could you please correct the error and replace the current cheque? I would like to get the repairs done as soon as possible and your cooperation in this matter would be highly appreciated.
