This is in reference to the seven days holidays that you have taken without any prior notice. You are reporting in the office after 7 days. This kind of irresponsible behavior is highly condemnable. Any kind of holiday that is taken without prior permission is not considered as a leave. Moreover it is considered as a willful negligence of duties.
You are working on a senior position. Your behaviour serves as a guideline to your team and other junior employees. And it is certainly not something that company wants you to give it to the rest of the employees.
Being absent from duties without prior consent is against the companies HR policies and demands immediate actions. The senior management had a meeting regarding this matter and had decided to consider this as first and last time mistake. In case it is repeated, serious actions would be taken.
Please submit an undersigned letter stating that such behavior won’t be repeated again.

Sample Memo for Negligence of duty, misconduct
It is reported against you that on 5th of March 2014 at 11 am you were found to be smoking in the office premises. As per the company norms, we follow No smoking policy which is not adhered by you intentionally. No employee or visitor is allowed to smoke in any work area whether it is private cabins, conference rooms or the toilets.
You are working with the company from last 5 years and it has been two years when the company enforced No Smoking policy in the office premises. Being in the company from such a long time, you are well acquainted with the company’s rules and policies. There is no reason that can justify your act. Moreover it is not the first time that you are caught violating the No Smoking Policy. It is third time in six months that the management has received a complaint against you. You are setting a bad example for the junior staff and the new employees. It is affecting the work culture negatively. A bad example can spoil the entire staff.
Seeing the severity of the matter, the management has decided to suspend you for a time period of 15 days. You are further advised in your own interest to be cautious and not to repeat such an act in future.
